I have no idea what happened. I tried to log in to reactivate my account, but log-in failed after putting in my authenticator code in. After three fails, my authenticator locked. Blizzard support - once I finally got through - unlocked it and I could log in. No obvious missing anything, or problems....

I IS UNLOCKED!

Authenticator = Good... except when it locks you out and you can't reach Blizzard Support to unlock it.
