Why is your character a Death Knight?
Razkiel is a death knight because he desired power and lingering around Northrend as a junior member of the cult of the damned wasn’t getting him where he wanted fast enough.
Did they fall willingly into the hands of the Lich King? The choice offered him was living service or they would kill him and he’d serve as a mindless ghoul. That’s not to say he didn’t serve willingly. There’s nothing entirely ignoble about his choice, but then again he wasn’t a nice or noble soul before serving either.
Are they alive or undead? Razkiel is definitely alive.
Do they gain their powers from a runeblade, or another source? He has training as a necromancer, so with runeblade or without, he has his powers. It is just that the runeblade is very intimidating.

I'm going to guess not, but should I ever finish the story, Brraaiins followed the call of the Lich King more out of curiosity to the voice in her head than an actual willingness to follow him.
Are they alive or undead?
Partially alive. I'm going to say she lingers somewhere on the verge where some systems are still operating, but really nothing too concrete. For all intents and purposes she's undead, being fueled by pure unholy energy.
Do they gain their powers from a runeblade, or another source?
This is a bit open for now. I'm going to say probably the runeblade, but if I can come up with a better idea I will.

Akeelah is a death knight because Setsana decided that she should be a death knight.
Did they fall willingly into the hands of the Lich King?
Nope. Akeelah was shot in the stomach by Setsana, and dragged to the Plaguelands to be found by the Lich King's minions. It was a long shot, as Akeelah was much more likely to be converted into a ghoul. Or just eaten.
Are they alive or undead?
Undead.
Do they gain their powers from a runeblade, or another source?
Akeelah's powers are enhanced by her runeblade, but for her they are merely her natural arcane powers twisted and again made stronger by her state of undeath.
